Don't Forget.....

All athletes will be required to complete all paperwork for fall sports before the first day of practice, July 31.  If all required paperwork is not completed by this date, the athlete will not be permitted to practice until it is completed.

Waynesville Middle School/High School Athletics will be using Final Forms this year for all paperwork.  Parents should create an account (if you have more than one child, you use the same account for all of them).  You will e-sign papers on here and fill out health history (so that important health issues show up on emergency medical forms).  Dates for yearly physicals for athletics will be added as they occur, so that you'll receive notifications when physicals are about to expire.

Courtney Powell, Waynesville's athletic trainer, will be available the week of July 24-28 to meet at the school for those that need access to a computer or that need help with signing up.  Otherwise, you may use the link below to get started.
