Seventh Grade Vaccination Requirements 2017

ATTENTION ALL CURRENT 7TH GRADE PARENTS!    Below are the 2017/2018 SEVENTH GRADE VACCINATION REQUIREMENTS:  The Ohio Department of Health has revised the School Immunization Requirements effective January 4, 2010. All students are to receive an additional dose of Tdap (Tetanus, Diphtheria and Pertussis) vaccine BEFORE entering the seventh grade. If your child has received a Tdap vaccine within 5 years of entering the seventh grade (2013 or later), this will meet the requirement, but verification is still required.   Additionally, beginning with the start of the 2017-2018 school year, one dose of meningococcal vaccine is required prior to entry into the 7th grade.   Please drop off a copy of immunization record to the middle school office ASAP.  There are various ways you may obtain these immunizations:   Your private physician   Warren County Health Dept., 416 S, East State Street, Lebanon, OH 45036, 513-695-1228   Pharmacy and store walk in clinics  If you have any questions please call me at (513) 897-4350 or email at jroyalty@wayne-local.com
